Pathway Grid

Pathway Grid is an operational control system for care pathways delivered outside the hospital. It shows whether the equipment, consumables and services required to deliver care are actually in place—before care begins or changes in the community, including but not limited to hospital discharge. By continuously learning from pathway performance, it helps health systems identify risks earlier, take faster action, and improve the reliability of care delivery across organisations.
